Service agreements and policies for LEAP Therapy Services

Service Agreement

By engaging LEAP Therapy Services for speech therapy and/or occupational therapy assessment and treatment, you agree to the following terms and conditions:

Services Provided

LEAP Therapy Services provides mobile speech therapy and occupational therapy services for children, including:

  • Comprehensive speech, language, and occupational therapy assessments
  • Individualised therapy sessions (speech therapy, occupational therapy, or both)
  • Written reports and documentation
  • Parent coaching and home program development
  • NDIS reporting and plan reviews
  • Collaboration with other professionals (with consent)
  • Multidisciplinary care coordination between our speech therapists and occupational therapist

Our Team

LEAP Therapy Services employs a multidisciplinary team of speech therapists and occupational therapists. Your child may be seen by one or more members of our team depending on their needs. Relevant clinical information is shared between team members involved in your child's care to ensure coordinated, effective therapy (see our Privacy Policy for details).

Appointments

Scheduling: Appointments are scheduled by mutual agreement. Regular weekly or fortnightly sessions are recommended for optimal progress.

Session Duration: Standard therapy sessions are 45-60 minutes. Assessment sessions may be longer (60-90 minutes).

Location: Sessions are provided at your home, child's school, daycare, or other agreed location within our service area (Annerley 4103 and surrounding suburbs). Telehealth sessions are also available.

Cancellation Policy

  • Notice Required: 24 hours notice is required for cancellations or rescheduling
  • Late Cancellations: Cancellations with less than 24 hours notice will be charged at full session rate
  • No-Shows: Failure to attend a scheduled session without notice will be charged at full session rate
  • Emergencies: We understand that emergencies and illnesses occur. Please contact us as soon as possible and we will work with you to reschedule
  • Therapist Cancellations: If we need to cancel or reschedule a session, we will provide as much notice as possible and will not charge for the missed session

Fees and Payment

NDIS Participants: Services are invoiced in accordance with NDIS pricing guidelines. Invoices are submitted directly to your NDIS plan (plan-managed or agency-managed) or to you (self-managed).

Private Clients: Fees are provided during initial consultation. Payment is due at time of service or within 7 days of invoice.

Payment Methods: We accept bank transfer, credit card, and direct debit.

Overdue Accounts: Therapy may be suspended if accounts are overdue by more than 30 days.

Consent and Confidentiality

All client information is kept strictly confidential in accordance with our Privacy Policy and professional ethics guidelines. Clinical information is shared between LEAP Therapy team members involved in your child's care to support our multidisciplinary approach. Information is only shared with external professionals or organisations with your written consent, except where required by law.

Reports and Documentation

Written reports are provided following assessments and as requested for NDIS reviews, schools, or other purposes. Additional fees may apply for comprehensive reports beyond standard documentation.

Professional Standards

LEAP Therapy Services and all team members operate in accordance with:

  • Speech Pathology Australia Code of Ethics
  • Occupational Therapy Australia Code of Ethics
  • NDIS Practice Standards and Code of Conduct
  • Privacy Act 1988 and Australian Privacy Principles
  • Relevant state and federal legislation

Limitations of Liability

While we strive to provide the highest quality services, we cannot guarantee specific outcomes from therapy. Progress depends on many factors including consistency of therapy, home practice, and individual child factors.

Termination of Services

Either party may terminate the service agreement with reasonable notice. We may discontinue services if:

  • Payment obligations are not met
  • Appointments are repeatedly cancelled or missed
  • The therapeutic relationship has broken down
  • Services are no longer appropriate for the client's needs

In such cases, we will provide referrals to other suitable providers.

Changes to Terms

These terms and conditions may be updated from time to time. Clients will be notified of significant changes. Continued use of services constitutes acceptance of updated terms.

Dispute Resolution

If you have a concern or complaint, please contact us first. We are committed to resolving issues promptly and professionally. If a resolution cannot be reached, you may contact Speech Pathology Australia or the NDIS Quality and Safeguards Commission (for NDIS services).
